If there is incorrect information listed for you within the directory, please click the 'Contact Us' button on this page.  This will bring you to the 'Submit a Request' page. Please be sure to select WebMD & Vitals Physician Directory/Reviews from the drop down. 

From there, please fill out all applicable fields. We need this information from each doctor separately. Multiple requests for several doctors are not allowed, and the request must come from the doctor themselves. 

Please make sure you have the following information to supply within your request:

1. NPI
2. First Name, Last Name
3. The URL where you see your listing on our website. www.doctor.webmd.com/.......
4. If you are requesting to change the address of a location, our system needs to have at least one location on file, so please always list the current address(s), along with the one that needs to be removed.
5. Adding a link to the practice as proof of locations or phone number will speed things along.
